Since all x-ray machines must be registered with the Department of Community Health prior to being put into use, a radiation machine registration application must be completed and returned to the Department with the appropriate registration fee payment. Furthermore, since the intended use of the x-ray machine is for mammography, a mammography machine authorization application must be completed and returned to the Department. Upon receipt and approval of the completed applications and registration fee, the Department will issue a radiation machine registration certificate, radiation machine tag, and mammography authorization to the registrant.
