What prevents the processing of transcript requests?
The following is a list of the most common reasons requests are not processed. • Request not submitted on Official Student Transcript Request Form • Incomplete request form • Missing Student Record/Index Number • Missing/incorrect Name or Name Used in School • Missing/incorrect Year Graduated or Last Year Attended • Missing/incorrect name of School Last Attended in LAUSD • Missing/incorrect Data of Birth • Missing Signature • Missing/incorrect fees • Request made to the incorrect location. Students should always first check with their school to determine whether their records are still maintained at the school. If the records were transferred to the Student Records Center, requests are made to the Student Records Center using the Official Transcript Request Form.
