When are Home Health Services usually needed?
• A patient is discharged from the hospital and home care will help prevent re-admission. • New diagnosis requires in-home teaching to reinforce physician’s plan of care, avoiding complications or unplanned hospitalizations. • Severity of illness at time of office visit requires assessment & monitoring a home to shorten recovery time or prevent hospitalization. • The patients medical condition is compromised by physical frailty, cognitive decline and/or social isolation and is best resolved by teaching family or designated caregiver’s in proper disease management and treatment techniques. The patient has an exacerbation or worsening of a medical condition requiring new medications or treatment.
