Where can I read the Faculty Governance about online course evaluations?

There are 12 university-wide questions asked on each evaluation; seven are quantitative; 2 are qualitative, and three are multiple choice. These 12 questions are required for all evaluations. In addition, departments can, if they choose to, ask additional questions on their evaluations; some departments have chosen to ask only the 12 university-wide questions.
