Which address(es) should I list for an Entity Name?
For each company or person you list under Entity Name in Section II, please provide a current business address and any different prior business address(es) used by the company or person during the time period for which you claim the contractual right to collect the market share, if the address is different than the address pre-printed on page 1 of the Late Fee Program Registration Form and/or the address you provided in Section I of the form.
