Who can fill out an application for Autism Waiver Services?
Parents can print and fill out the application at http://www.srskansas.org/hcp/css/Autism/Autism_Preliminary _Application.pdf. Where to send the application is printed on the application. Then if the child meets the criteria for the Autism Waiver, you will receive a letter stating they have been placed on the Proposed Autism Waiver Recipient List and their numerical number on the list. When a position becomes available you will be contacted by the Program Manager to offer you a position.
