Who is responsible for payment?
The patient or legal guardian is ultimately responsible for costs incurred for care. Once the appropriate forms are completed, The University of Toledo Medical Center will make every effort to collect from your insurance companies or government agencies. It is your responsibility to contact your insurance companies to ensure prompt payment.
If you are 18 or over, you are legally responsible for your own account, regardless of who you live with, who claims you as a tax deduction or who has the contract with the insurance company. If the patient is under 18, parents OR the legal guardians are responsible for payment. You are fully responsible for knowing the specifics of your particular insurance coverage. Examples of these specifics include: co-pays, deductibles, second opinions, pre-authorizations, preferred providers, referrals, covered and non-covered services and preferred hospitals and facilities. Our office has been instructed to request pre-authorization from your insurance company for your hospital and or surgical visits. Please note that this does not guarantee payment. Payment is made according to your contract. The clinic cannot accept responsibility for collecting your reimbursement or negotiating a settlement on a disputed claim. We can provide you with the necessary medical information to assist you. Back to
