For changes in the start/and or stop date(s) the Environmental Health Notifications Group (EHNG) in the central headquarters office in Austin and the appropriate DSHS Regional Office must be notified by telephone between 8:00 a.m. and 5:00 p.m. Central Time (CT) prior to the original start and/or stop date(s) as previously specified on the notification form. A written amended notification must be postmarked within 24 hours following the telephone communication with the EHNG and the regional office.
