CareSource has been supporting independence through excellent care for more than 10 years. We are an affiliate of the Visiting Nurse Association of South Central Connecticut, a local non-profit, community based organization with more than 100 years of history serving individuals of all ages and their families in Greater New Haven, Milford and the lower Naugatuck Valley. CareSource is accredited by CHAP, the Community Health Accreditation Program, and has a professional Advisory Committee that oversees its clinical practices. At the time of admission, a nurse manager comes to your home and develops a care plan with you. The nurse manager orients the staff to your plan of care and performs routine supervisions.
