Why do I need a Practitioner Code to make a SmartForme order?
SmartForme products are specifically for bariatric surgery patients. The Practitioner Code ensures that you are eligible to order SmartForme products. Per our agreement with an outstanding community of bariatric physicians, surgeons and dietitians—during the order process, customers are required to provide a pre-assigned code or complete practitioner information. In many cases, your practitioner is already recommending SmartForme products to their patients. If not, when a new customer fills out a Practitioner Code Request or Contact Us Form, we routinely send the practitioner a sample and information packet as a courtesy and issue them a Practitioner Code for patients under their care.
