Why doesn the stand-alone PDF form include a credit card field?
The PDF form provided here can be filled out on screen and then printed using Adobe Acrobat Reader. This form is provided as a convenience for our customers who prefer not to place orders online. It is not intended to be filled out and emailed or otherwise electronically submitted. In order to prevent a credit card number from being inadvertently sent unencrypted over the internet or stored on a hard drive, there is no credit card field in the form. Once you’ve completed all of the other needed fields, please print the form and fill in the credit card number by hand then fax or mail the printed form to the address at the top of the form.
