A. Set up the question as a Matrix Row Question and have only one row. To do so: • On the Question Type tab, choose Response Type 8: Matrix Questions and then choose Matrix Type 4: Row Questions. • On the Response Options tab, click on Allow Multiple Answers. • In the Matrix Column Response Codes and Labels field, enter your response/answer options • Leave the Matrix Row Labels field blank (you may want to enter a single blank space on the first line of this field). This should give you a table with the “check boxes” side-by-side and the Answer labels above each check box.
