What forms need to be filed in order to graduate?
The Degree Audit form is available from the Registrar’s Web site. Download and print a copy of the one for the semester you entered the Physics department. Fill this out and take it to your advisor. He will sign it and send it to Degree Services. It would be a good idea to print out a copy of your on-line transcript and take it with you to your advisor. Any request to change graduation requirements from those on the Degree Audit form must be on a “Petition to Alter Degree Requirements”. A copy of this form is available from Dr. Gary Agin, 103 Fisher. An application for graduation must also be filed. This is available from the Degree Services office in the Administration Building.
