What is the difference between assessment data and cues?
During the assessment phase, the gathers a lot of data from observing, inspecting, examining and listening to the client. The nurse uses critical thinking skills to select from all the data certain pieces of data that are of importance to the client’s condition. This is a skill that takes time and clinical experience to develop.The student of nursing takes extra time to review the data, compare subjective and objective data and to search for patterns in the data. Care is taken to be sure that data from all sources: interview, health history, symptoms, physical exam lab and diagnostic data are examined and included in the decision to identify a cue.
